The transfer note is issued by the college where the pharmacist is registered, based on a request. Before the transfer note is issued, the requesting pharmacist is obligated to pay up to date the dues owed to the college they wish to leave. The transfer note will be handed, with signature, to the applicant. In the register of pharmacists (https://portal.net-dd.com), the college to which the pharmacist has transferred will be mentioned. If a complaint has been filed against the pharmacist at the college from which they are transferring or a disciplinary investigation has been initiated, the complaint or the disciplinary file will be forwarded to the college to which the pharmacist is transferring.
